Thank you for becoming an SMDCard Member! We provide free SMDCard membership to all single moms; just fill out the form below. Once we receive and process your information, we will email you a SMDCard (in JPEG form) and will send you a plastic card/key card SMDCard in the mail to use with participating businesses.
